These Terms of Service ("Terms") govern your access to and use of PharmacopeiaMD (the "Service"), operated by ATX Infusion and Wellness d/b/a PharmacopeiaMD ("we," "us," "our"). By subscribing to, accessing, or using the Service, you agree to these Terms. If you do not agree, do not use the Service.
The Service is intended for licensed healthcare professionals (including physicians, physician assistants, nurse practitioners, and pharmacists) and for students in accredited health-professional programs. You must be at least 18 years old. You represent that any professional or student status you provide (including for discounted pricing) is accurate, and you agree to re-verify it upon request.
The Service is a clinical reference and educational tool. It presents drug, condition, dosing, and related information to support—not replace—the independent professional judgment of a qualified clinician. See the Medical Disclaimer in Section 2, which is incorporated into these Terms.
You acknowledge and agree that you retain sole and full professional responsibility for all clinical decisions, diagnoses, prescribing, dosing, and patient care undertaken by you, including any decision informed in whole or in part by the Service. You are responsible for independently verifying all information (including doses, interactions, contraindications, and indications) against current, authoritative primary sources and product labeling before acting on it.
We grant you a personal, non-exclusive, non-transferable, revocable license to access the Service for your own professional or educational use. Access is granted through an individual access code or account. You agree not to share, resell, sublicense, or distribute your access code or account, and not to copy, scrape, redistribute, or create derivative works from the Service's content. We may suspend or revoke access that we reasonably believe is being shared or misused.
The Service is offered as an annual subscription that renews automatically each year at the then-current price, charged to your payment method through our payment processor (Stripe) until you cancel. You may cancel at any time; cancellation stops future renewals, and access continues until the end of the paid term. Except where required by law, payments are non-refundable. Discounted (e.g., student) pricing may require annual re-verification and will revert to standard pricing if eligibility is not maintained.
THE SERVICE AND ITS CONTENT ARE PROVIDED "AS IS" AND "AS AVAILABLE," WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, ACCURACY, COMPLETENESS, OR NON-INFRINGEMENT. We do not warrant that the content is error-free, current, or suitable for any particular clinical situation.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, WE WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR FOR ANY CLINICAL OUTCOME, INJURY, OR LOSS ARISING FROM OR RELATED TO YOUR USE OF, OR RELIANCE ON, THE SERVICE. OUR TOTAL AGGREGATE LIABILITY FOR ANY CLAIM RELATING TO THE SERVICE WILL NOT EXCEED THE AMOUNT YOU PAID US FOR THE SERVICE IN THE TWELVE (12) MONTHS PRECEDING THE CLAIM.
You agree to indemnify and hold harmless ATX Infusion and Wellness d/b/a PharmacopeiaMD and its owners and affiliates from any claims, damages, or expenses arising out of your use of the Service or your clinical decisions.
All content, design, and materials in the Service are owned by ATX Infusion and Wellness d/b/a PharmacopeiaMD or its licensors and are protected by intellectual-property laws. No rights are granted except the limited license in Section 1.4.
We may update the Service or these Terms from time to time. Material changes will be reflected by the "Last updated" date above; continued use after changes constitutes acceptance.
These Terms are governed by the laws of the State of Texas, without regard to conflict-of-law principles.

This reference is intended as a general guide to therapy only. The Service does not establish a provider–patient relationship, does not provide patient-specific recommendations, and must not be used as the sole basis for any diagnosis, prescription, or treatment. The treating clinician is solely responsible for all patient care decisions. Do not enter protected health information (PHI) or patient-identifiable information into the Service.
In an emergency, rely on established emergency protocols and clinical resources, not on this Service.
We collect the information you provide when you subscribe or contact us: your name, email address, and professional role/status. Payments are processed by Stripe; we do not receive or store your full card details—Stripe handles payment data under its own security and privacy terms.
We do not collect patient health information. The Service is a reference tool; you should not enter patient-identifiable data or PHI into it.
We use your information to provide and deliver access to the Service (including issuing and managing your access code), process payments and renewals, send transactional messages (receipts, access, and account notices), verify eligibility for discounted pricing, and—if you opt in—send occasional product updates. You may opt out of non-essential email at any time.
We share information only with service providers who help us operate the Service (for example, our payment processor and email provider), and as required by law. We do not sell your personal information.
We retain your information for as long as needed to provide the Service and meet legal and accounting obligations, and we use reasonable measures to protect it. No method of transmission or storage is completely secure.
You may request access to, correction of, or deletion of your personal information, and you may unsubscribe from marketing email, by contacting support@practiced.health.
